Please be advised that in order to file a claim under the Camp Lejeune Justice Act, we will need a notarized affidavit as well as a DD 214. The affidavit will require that you specify the dates spent at or near Camp Lejeune as defined by the Camp Lejeune Justice Act, as well as the cancer(s) and/or condition(s) you believe you or a loved one suffered as a result of the water contamination. Please be aware that the affidavit will require that you certify under penalty of perjury that the facts are accurate. In certain cases, we may require at least some medical records. We have the ability to obtain these records, but it can be a timely process. As the deadline approaches, we will require you to produce these documents in order to file your claim.

Please also note that if you are looking to file a wrongful death claim on behalf of someone who is deceased, an estate must be opened. This process can take several weeks, if not longer. Given the approaching deadline, we reserve the right to require that you provide proof that an estate has been opened before we file your claim. We will review these on a case-by-case basis, but we cannot guarantee that we can file a claim if an estate has not been opened.
